Before applying for a OnePay Debit Card via Leeds Credit Union, please read the following information carefully and tick the box to confirm that you understand and accept the points below.

I understand and acknowledge the following:

• The OnePay Debit Card is a pre-paid card and not a credit product;
• There is a monthly Card fee and other charges may apply depending how I use the OnePay Debit Card;
• The OnePay Debit Card is NOT a Leeds Credit Union product. By applying I will become a customer of Modus UK Ltd, trading as OnePay;
• I have read the OnePay Debit Card Terms & Conditions.
• I am responsible for the use and security of the OnePay Debit Card.
• Any queries, issues or complaints about the OnePay Debit Card must be directed to OnePay, not Leeds Credit Union.
• I understand that there are other prepaid debit card products available. I am happy to apply for the OnePay Debit Card and confirm this is my choice.
